Our Approach to Care Coordination
At Shared Smile Behavioral Health PLLC, we believe that high-quality psychiatric care includes thoughtful collaboration with other professionals involved in your care. When appropriate and with your permission, we coordinate with your healthcare providers to support a more comprehensive and integrated approach to your mental health.
What Care Coordination Includes
Care coordination may involve:
- Communication with your primary care provider (PCP)
- Collaboration with therapists, psychologists, or other mental health professionals
- Coordination with specialists involved in your medical care
- Reviewing relevant medical records or treatment plans
- Providing updates or summaries of care (with your authorization)
Your Authorization
Your privacy is a top priority. We will only share your information with other providers if you give written consent, unless otherwise required or permitted by law.
You have the right to:
- Approve or decline care coordination
- Specify which providers we may communicate with
- Revoke your authorization at any time

Fees for Care Coordination

While we strive to collaborate effectively with your care team, we cannot guarantee the availability or responsiveness of external providers.
